Parent & Guardian Sections
Parent/LEGAL Guardian Demographics
Enter each parent or legal (court-appointed) guardian or caregiver (with affidavit) of the student, even if a parent/guardian lives at a different address than the student. Do not enter siblings or other household members in this section.
- Use legal names that would be on a driver’s license or other legal document. When a Parent/Legal Guardian picks up a student from school, legal identification will be required, and the names will need to match. However, you may enter a nickname or preferred first name for communication, if you wish. Leave the preferred name field blank if there is none.
- If the Parent/Legal Guardian does not live at the student's primary address given in the previous section, you may optionally enter location information or a U.S. mailing address where school mailings can be sent. (No materials are mailed outside of the U.S.)
- Each Parent/Legal Guardian's highest education level is required by the state.
Parent/LEGAL Guardian Contact Information
Please provide as much Contact Information as possible, so we have the best chance of contacting a Parent/Guardian when necessary.
- Emails should be unique between parents/guardians if both want a Schoology account for their student(s), since Schoology uses an email as the account username.
- You may un-tag any of the preferred contact preferences if this email address should not receive emails for any of the reasons listed.
Parent/Guardian Active Military Service
The U.S. Federal government requires that we collect information on Parents/Guardians in active military service as part of the Every Student Succeeds Act (ESSA).
Household Information Sections
Primary/Home Phone Number
Enter the phone number that will be the primary point of contact for a student and his/her family. This can be a home land line number or a parent/guardian cell number. (Each parent/guardian’s cell number and work number will be requested later on as well.)
Home Address
Enter the primary home address for the student. The student will only be associated with one address, even if he/she has residence at multiple addresses.
Start by entering the house number. Continue with the first few letters of the street name, until your address is displayed in the Address List which will pop up below where you are typing. As soon as you see your address appear in the Address List, click on the address; the address fields will fill for you.
Reasons your address may not display in the Address List:
- Your address is within the boundaries of FUHSD, but may not have made it into our system yet.
- Your address is not within the boundaries of FUHSD, so is unlikely to be in our system.
If your address does not display in the Address List, you will need to contact the FUHSD Enrollment Office (408.522.2266) to resolve issues with addresses and school boundaries.
HoUSEHOLD INCOME
The number of people in the household refers to the household group that lives together at the given address. It should not include a parent who lives elsewhere nor members of an entirely separate family living at the same address.
Your actual income is not requested - only below a threshold, between thresholds, or above a threshold. Providing this information can help FUHSD secure funds from local, state, and federal agencies. It may also benefit your family directly.
Emergency Contact Sections
Emergency Person Demographics
In an emergency, the Parents/Legal Guardians are called first, then the Emergency Contacts if a guardian cannot be reached. You must enter at least one (two are preferred) Emergency Contact who is not the parent/legal guardian. This person must be at least 18 years old with a valid phone #.
Enter the legal first name and legal last name of this person. In an emergency, students can only be released to someone proving their identity with a legal document, such as a driver’s license. Use the Nickname field to indicate a different/preferred first name, if desired; otherwise, leave the Nickname field blank.
Please provide the middle name, suffix, and birth date, if possible, so we can distinguish this person from others in our system with the same or similar names.
Check the box indicated if this person is likely already in our data system because he/she is a current or former student, parent, emergency contact, staff member, etc.
Emergency Person Contact Information
Enter phone numbers and an email where the person can be reached. At least one phone number is required. (If phone numbers or email address change, these data may be updated throughout the year through the Parent Portal into our data system. You will get more information on the Parent Portal after your student is enrolled.)
Student Information Sections
PREVIOUS SCHOOL INFORMATION
These questions pertain to schools your student attended in grades Kindergarten through 12th grade, so do not include nursery school, pre-school, or Pre-K schools in these answers.
- First Entry dates: If you do not remember the exact start date of the student’s first (K-12) school year in the US or in CA, you may use Sept. 1 of the correct year.
- Most Recent School: We need information about the most recent school the student attended, so we know where to ask for grades, records, etc. (We realize this could be duplicated info if your most recent school was a CA school and you just entered that CA school name. If so, please enter it again here anyway.)
- Entry Situation: Please choose the scenario which describes your student's upcoming entry into FUHSD. This is important to help us collect the correct documents needed for entry into our district.
- Recent Expulsion: If the student has been expelled within the last two years, please enter the grade or grades when this occurred.
HEALTH/MEDICATION INFORMATION
Enter any health conditions or medications for your student. A link is provided so you can download the required form which will need to be filled out by you and your doctor. Doctor contact information is optional in this section.
Release agreements and policy acknowledgements
You will be required to click on the links to all policy notices and indicate your compliance. You can download any of those documents; they are all also available on the fuhsd.org website.

You will be required to upload four residency documents which list the student's address and parent/guardian name:
- Parent/Guardian government-issued picture ID
- Recent utility bill or statement
- A vehicle registration or a payment document (e.g., current paystub, most recent W-2 tax form)
- Most recent property tax bill or tenant's copy of lease agreement that also shows owner/manager's name & phone

Digital Address Verification
To simplify the enrollment process, this district is providing you with a document-free primary address verification option. This simple process should take less than a minute. For best results, please use the information of the Parent/Guardian whose name is most likely to appear on your household bills and other official documents.
Step 1: Reverse cell phone authentication
Simply enter your name (First and Last) and your cell phone number, then hit SEND button. You will be sent a one-time cell phone authentication code to the cell phone number you entered.
This step verifies that the owner of the cell phone provided is the person submitting the information. Your cell phone number will only be used for the purpose of authentication and will not be used for any other matter without your explicit consent.
If you want to bypass the electronic verification process and upload all required address verification documents, you may click on the SKIP VERIFICATION button. Please note, because the Enrollment Office staff will need to manually verify each address document, your registration will take longer to process and more time will be spent verifying your primary address.
Step 2: Enter cell phone authentication code
Check your cell phone and enter the authentication code sent to you. Enter the correct authentication code, then click on SUBMIT. If the proper authentication is entered, you will go to the next step to verify your primary address.
Step 3: Primary Address Verification for Enrollment & Registration
Enter your Primary Home Address and click on VERIFY ADDRESS. The system will complete an online primary address verification to verify the location of your primary address and to check if the address is within the Fremont Union High School Attendance Boundary.
Once completed, the system will return a 7-digit ENROLLMENT ACCESS CODE.
This access code is required to begin your On-Line Registration (OLR). Please write down this code or take a picture of it with your phone. You will be required to enter this code as the first step of completing your registration.
Once you write down or copy the code, click on the BEGIN ON-LINE REGISTRATION to continue with your student’s registration and enrollment for the 2025-26 school year.
