2008-2009 REGISTRATION &
RESIDENCY VERIFICATION PROCESS
FOR STUDENTS NEW TO THE DISTRICT

All NEW INCOMING STUDENTS from private schools or other school districts must register at the Fremont Union High School District's Enrollment & Residency Office located at the District Office, 589 W. Fremont Avenue in Sunnyvale
8am to 4pm, Monday through Friday 408-522-2200

IMPORTANT NOTICE:  Students from feeder middle schools who are participating in the residency verification process at District high schools should follow the procedures mailed in the January registration packet.  All other new incoming students should follow the procedures delineated below.  NOTE: incoming private school students register after February 25, 2008.

RESIDENCY VERIFICATION DOCUMENTATION:
All new and continuing students must provide residency verification documentation.  A student can have only one residence for the purposes of establishing residency.  A student shall be considered to have complied with residency verification requirements when parents/legal guardians have provided the following documentation:

BIRTH CERTIFICATE OR PASSPORT & IMMUNICATION RECORDS

plus THREE (3) DOCUMENTS REQUIRED TO ESTABLISH RESIDENCY —
one from each of #1, #2 and #3 below.

Post Office boxes do not meet residency requirements.  Only street addresses are accepted.

Utility (PG&E, water, etc.), telephone bills, or deeds are NOT VALID
residency verification documents.

      #1 – Parent/Guardian’s Picture ID from the following list:  Current California State
               Driver’s License; OR Current California State ID Card; OR Valid Passport of Consulate
               Issued Picture ID; OR Credencial Para Votar. 

      #2 – ONE of the following ORIGINAL DOCUMENTS with the parent/guardian’s name
               and address: 
Current valid vehicle registration; OR Current (2007) W-2 forms; OR
               Current paycheck issues by employer with both the employer and employee’s name
               and address imprinted on the paycheck.

      #3 – ONE of the following ORIGINAL DOCUMENTS with the parent/guardian’s name
               and address:
 Original Property Tax Bill with parent name and current 2007-08
               property address, indicating homeowner’s exemption; OR Tenant original copy of
               Rental or Lease Agreement with parent/guardian name, student name and address, as
               well as manager or owner’s name and phone number. 

If it is determined that false information was given to secure or to determine residency,
the student will be disenrolled.

CAREGIVER AFFIDAVITS:
Students shall qualify as District residents if they reside FULL-TIME in the home of a caregiving adult within District boundaries.  The caregiving adult must provide residency verification as outlined above.  “Full time” for the purposes of this regulation is defined as:

      1.  The student’s primary residency is that of the caregiving adult, 24 hours a day,
            seven (7) days a week
      2.  The student resides in the home of the caregiving adult during periods of
            vacation or repose.

Residency verification for students on Caregiver Affidavits is handled at the District's Enrollment & Residency Office only.  Please contact the Residency and Enrollment Office at (408) 522-2266 to arrange an appointment to meet with a District administrator.  

If it is determined that false information was given to secure or to determine residency,
the student will be disenrolled.

INTERDISTRICT ATTENDANCE AGREEMENTS: 
Due to space and enrollment considerations, Fremont Union High School District limits the number of incoming interdistricts.  If you believe that you have a unique circumstance that qualifies your child for attendance in our District on an interdistrict transfer, you may request a transfer from your resident district. Release from the resident district is required and the conditions under which the transfer is requested must be clearly stated.  Given that many of our schools are at or nearing capacity, approval by the Fremont Union High School District is not guaranteed. 

If it is determined that false information was given to secure or to determine residency,
the student will be disenrolled.
